Place all information in a 3 ring binder.

Include copies of coaches information.

Place each players information in a clear document protector.

Players should be in alphabetical order.

Place the pink notarized medical form on top.

Place the purple KSYSA player registration form on bottom.

Place the copy of the player's birth certificate in the middle.

Place pictures (if included) in an envelope also in the middle.

Checks should be made out to the Flint Hills Soccer Club.

One check for carding fees.

One check for Club fees.
